From what I have read all over the Net Nissan has been less than cooperative with fixing these issues with proper solutions. should I expect the worst and plan on my truck being in and out of the shop constantly as others are?

The brake judder is an example of how Nissan stepped up to the plate and provided a free upgrade to a better design. I use my new brakes hard and constantly on the curvy roads I drive, and they have something like 43K miles on them.
Rear differential. Almost all issues are with the spider gears. Near the end of '05 they upgraded the OR Titans to a 4 pinion vs. the original 2 pinion, so this should help. Many tens of thousands of Titan owners have zero differential issues, although a couple of people that I have a lot of respect for have had issues ... it happens. I think the chance of it happening to any owner that drives like grandma are statistically low, but if you are near your 60K mile drive train warranty expiration, or 100K for a extended warranty, consider sacrificing your locker and buying a Tru-track limited slip differential.
I might do this at just under 100K miles, as insurance, but mainly because it is always available ... any speed, any mode, i.e. 2wd, 4HI, 4LO. The locker is amazing but idling up mountains is sort of boring, scary at times, but I like faster off roading.
