All they do at a minimum (if anything) is tell the ECU that the air moving through the MAF is colder than it really is by manipulating the voltage signal (lowering it). The reason they suggest running premium fuel is that when the ECU thinks it is getting colder air, it leans out the fuel mixture.

Does anyone know if this might effect the amount of timing the ECU adjusts for? That could be another reason for the premium fuel.

I don't think I'll be hacking into *my* wiring for something like this, but that's pretty much what they do.
