I replaced my stock muffler with a Flowmaster Delta Flow 40 single in/single out. I have the pipe turned down right behind the muffler. I paid $112 for the setup.
I am disappointed in the sound inside the cab. Not very loud at all in my opinion. It sounds ok from the outside but could use some improvement.
Any suggestions on what I could do to make this thing louder both inside and out?

get dual cut outs in place of the resonators, trust me it will be really loud. I have had this setup for a while now, and i had only one cutout in front of the muffler earlier, and before that i had a 40 series muffler on. i love the dual cutouts they are really loud and they are always open.

Straight pipes in lieu of the secondary cats/resonators PLUS dual glasspacks.
(Psssst: if you want LOUD, this should do the trick)
They are the next best thing to Zero mufflers!!
have the resonators cut off of your truck and have straight pipes put in in thier place this should double your sound easily..
my truck is a 2wd.....so does that mean I have 2 cats and 2 resonators? Are the resonators muffler looking objects right before the Y-pipe? Where are the cats located?

You're right. 2x2 Titans have two cats (welded to the exhaust manifold) and resonators which are the "muffler looking" objects as you described located downstream of the cats.
You want loud and raspy?
Replace these resos with a straight pipe.
