I bought a 2004 nissan Titan when they first came out in 2004 brand new. Im going on 250,000 miles and I am overheating I never had this problem before. I changed out the radiator, fan clutch and purged the cooling system. Once i finished I took it out for a test drive and overheating again I also switched out my cooling temp sensor and still overheating. I am not sure what to look for next but if anyone has ideas please feel free to send them my way.

Cooling isn't my area of expertise (frankly, I don't have an area of expertise...) so hopefully someone has experienced something similar.

I know you said you purged the cooling system, but did you just let it drain or did you purge it under pressure? Have you pressure tested it? It looks like you changed out pretty much every vital part of the cooling system, so the only thing that stands out to me is perhaps there's some type of blockage or buildup in your hoses, perhaps you have a bad thermostat, or it could be a bad radiator cap, unless you were just unlucky enough to swap one bad radiator with another bad radiator (been there, done that).

Under what conditions is it overheating? Just idle/stop and go traffic? Is it alright at highway speeds with ample airflow? Does it stabilize at a certain temperature, or just keep climbing until you have to shut it down?

I am over heating in stop and go traffic and also on the freeway I had to pull over and shut it down the first time. I did a pressure test and it came out normal and also checked the heads to see if they went bad but that was also normal. I did not change out my radiator cap or my pressure cap from the over flow tank. That will be my next step

I had a Pathfinder that was overheating and it ended up being the temperature sending unit. When it showed overheated, I put a thermometer in the coolant and it was fine. It never actually overheated. When yours overheats, is it blowing out steam, or are you just getting the indication on the dashboard? If it's actually overheating, might be the waterpump. 250k is a lot of miles for a pump. Good luck and let us know what you find.

Are you sure the cooling system is full?

Possible head gasket failure. Check for air bubbles in the radiator while the engine is running. Is the coolant level still correct?

I was thinking the same thing. When you say you "checked the heads" did you do a leakdown test on them or just a compression test or what?

I get the reading on the dashboard and im not getting no steam just the needle go way up. I replaced the temp sensor is that the sending unit.
Yes that is it. Sorry if I missed that. Also, are any of your other gauges acting kooky? There's a little voltage regulator in the dash that will go bad but it generally affects more than one gauge.

250,000 miles and overheating... The old Master mechanic I learned from would tell you to check the idle oil pressure with a gauge, IF, IF, nothing else like bubbles in the radiator or a compression test shows all-good.

If the idle oil pressure is low, then it's probably time to drop the oil pan and have a look at the bearings.

If you replaced all the above. I would check the thermostat. Could be stuck closed.
