After numerous attempts of having local shops and dealer diag and replacing a fuel injector front O2 and swapping out plugs and smoking the intake, etc.... It ended up being the rear O2 on bank one. Even though this code says bank on sensor 1 the rear wouldn't go above 3v. I hope this helps someone b/c it cost me over $500 in swapping parts and labor and a lot of forum searches and a lot of people seem to have this problem and have had a hard time correcting this issue.

My rear sensors are turned off by uprev, but I still throw this code. It's been 18 months..

I'm having the same problem for a few months after JBA & Cajun install and have thrown a bunch of parts at it including all the sensors, I have monitored with Cipher the rear 02 sensor starts around 30ish with bank 2 rear then after driving it drops to 0 and stays there =/ can you elaborate in as much detail as possible what caused the bank 1 rear 02 sensor to do this and what its part was in causing the PA200
