I would start with local places closest to you and just call them. It's pretty basic stuff and I couldn't imagine any place with a sign and a few wrenches couldn't get it done.

That video is actually for the 2" lift. The 3" lift includes the UCA's.

Leveling kits are easy to do but if you really don't wanna do it yourself theres a guy on this forum who does lift installs for a extremely low price. I had to have him do my leveling kit, uca and endlinks because I live on base and didn't have the tools to do it at the time. He does really good work and his name is Nick. Just search ups and down and you will find his thread on it.
