Titan won't rev past 4500

While I'm driving so far 15mph all the way to 90 mph when I step on it it will bounce off 4500 rpm. When I'm in park and neutral I can rev it up to 6000 no problems sounds heathy and everything. I was going into the high way and I stepped on it all the way down and it hit 5000 and just bounced back to 4500 and it kept bouncing. Runs smooth down the road and everything. I just got it back from the shop I had a skip in cylinder #6 shop replaced injector and it worked fine until now. I have no check engine light shakes or vibrations normal MPG drives around town like a dream just when I floor it. Every time this thing ends up in the shop I spend 500+ bucks. 149k 4 door SV help please.

Check the fuel pressure when under heavy load. If it falls below 45 psi replace the fuel pump.

Sounds like fuel delivery is a problem. What about fuel filter?
